Home Resource Facilitating Postsecondary Success: Strategies to Remove Service Roadblocks for Students with Disabilities
Individualized Education Programs (IEPs) are developed to provide students with disabilities the support they need, including a section on secondary transitions to help them prepare for life after high school. This section is crucial for guiding students toward their goals in education, employment, and independent living after graduation. However, navigating these postsecondary pathways can be challenging for both students and their families, as well as for the staff supporting them.
This infographic highlights these challenges and offers strategies that school staff and other agencies can use to make student transitions smoother and more successful. It also outlines five key practices that can help prepare students with disabilities for college, careers, and active community participation.
